The Men's Declaration should be filled out by men whose unborn child has been aborted. To fill out and submit a Declaration, simply download the form and print it out.  Please answer all questions as completely as your memory allows.  You may leave blanks. We realize this may be a difficult process for you, and we thank you for sharing your experience. The declarations will be used as evidence in legal efforts to end legalized abortion. Please type or print clearly all of the information. You may attach additional pages if more space is needed.
